Passports
The friendly and knowledgeable staff at the Recorder's Office can assist U.S. citizens, who meet the passport requirements, with obtaining a U.S. Passport Book or Passport Card.
Due to the Western Hemisphere Travel Initiative, it is necessary to make an appointment for the passport application process. Our agent is available Monday through Friday from 8:30 am to 3:45 pm. Please call 563-652-2504 to schedule your appointment.
The passport applications are available in our office or may be accessed by visiting Travel.State.gov. If printing your own forms, they must be presented single-sided.
Jackson County Recorder Passport Book/Card Checklist
1. Call Ahead for an Appointment - 563-652-2504
- To ensure our one Passport Agent is available at the time you want to apply
- Appointments are accepted from 8:30 am to 3:45 pm
2. Passport Application
- Print legibly in black ink
- Do not sign the application
- If you make a mistake, a new application must be completed
3. Passport Photo
Office personnel can take your photo at the time you apply.
4. Provide US Citizenship Evidence
Evidence must be an original or certified copy. The evidence will be returned to you by Passport Services.
Must submit one of the following:
- U.S. Birth Certificate that meets the following requirements:
- Issued by either a county or state of birth, not a hospital
- Lists your full name, date of birth, and place of birth
- List parent(s) full name(s)
- Has the date filed with registrar’s office (must be within one year of birth)
- Has the registrar’s signature
- Has the seal of the issuing authority
- Fully valid, undamaged U.S. Passport (may be expired)
- Consular Report of Birth Abroad or Certification of Birth
- Certificate of Naturalization
- Certificate of Citizenship
5. Present ID
Applicant and parents/guardians must present one of the following to acceptance agent:
- In-state, fully valid driver’s license
- It must be the permanent plastic ID, not the temporary paper copy
- Fully valid, undamaged U.S. passport (may be expired)
- Certificate of Naturalization or Citizenship
- Government employee ID (city, county, state or federal)
- US military ID or military dependent ID
- Valid foreign passport
- Matricula Consular (Mexican Consular identification commonly used by the parent of a US citizen child applicant)
6. Fees
Passport Services requires their payment to be a check or money order.
- One check or money order for each application
- Recorder fees can be check, money order, cash, or debit/credit card (3% fee)
7. All Applicants Must Apply In Person
This requirement also includes children.
8. Parental Consent
- Required for minors
- If the applicant is 16 or 17:
- Applicant and one parent must appear in person and present their IDs
- If the applicant is under 16:
- Applicant and both parents must appear in person and present their IDs
- If this is not possible, please ask for additional details
- Applicant and both parents must appear in person and present their IDs

Passport Book Fee
- Age 16 and over:
- $130.00 - Payable to US Department of State
- $35.00 - Execution fee to Jackson County Recorder
- $15.00 - Photo fee
- Age 15 and under:
- $100.00 - Payable to US Department of State
- $35.00 - Execution fee to Jackson County Recorder
- $15.00 - Photo fee
Passport Card Fee
- Age 16 and over:
- $30.00 - Payable to US Department of State
- $35.00 - Execution fee to Jackson County Recorder
- $15.00 - Photo fee
- Age 15 and under:
- $15.00 - Payable to US Department of State
- $35.00 - Execution fee to Jackson County Recorder
- $15.00 - Photo fee
